Fill out the forms.
It is a requirement that the application for Delivery of Mail Through Agent be notarized (Field “Signature of agent/Notary Public”) if you will not be signing this form at Brentwood Mailbox. (Out of Country applicants please email us for instructions on how to complete this part).

Mail in the forms. To expedite your order, fax the forms before mailing them. Upon receipt of fax, we will contact you with address information so you can get started.
